San Carlos Apache Healthcare Skilled Nursing & Long Term Care Facility

The Skilled Nursing and Long-Term Care Facility for the San Carlos Apache Healthcare Corporation is a 130,000 SF, single-story facility designed to serve the elder community of the San Carlos Apache Reservation. The facility includes long-term care, assisted living memory care, skilled nursing, and hospice services. Organized into four residential “neighborhoods” surrounding a central community center, the layout provides privacy while also fostering patient connection. The neighborhoods accommodate 60 long-term care units, 20 memory care units, and 20 skilled nursing units, including four hospice rooms. Each neighborhood features dedicated outdoor courtyards, shared amenity spaces, and support areas to enhance resident comfort and quality of care. The central community center offers a range of services and amenities including a large multipurpose room, café, fitness room, library, and worship space. Built with a strong emphasis on community, care, and cultural sensitivity, the facility supports both clinical needs and social well-being for residents, families, and staff.
